Nineteen of our 54 products carry a chip that says some version of "alcohol-free." On 16 it reads "Alcohol-free." On two it reads "Alcohol-free (no ethanol)." On one, hatherine's pore-melting nose pack, it reads "Denatured-alcohol-free." We sat down this week expecting to catch ourselves out on at least two of them.

We didn't. Re-counted today, the number of products carrying an alcohol-free chip that also contain ethanol, alcohol denat. or t-butyl alcohol is zero. The two we had flagged internally contain cetearyl and stearyl alcohol, which the FDA's own labelling page says behave differently on skin from the alcohol people are actually worried about.

So this stopped being a gotcha. The labels hold up. The wording is still confusing, and underneath it sits a mess we can't blame on anyone else: across seven products we spell one class of ingredient five different ways.

Nineteen of 54 products claim alcohol-free and not one contains a volatile alcohol, so on the narrow question our labelling is consistent. The two that made us nervous contain cetearyl and stearyl alcohol, fatty alcohols the FDA explicitly separates from ethanol, and we amended both chips on 16 August to read "Alcohol-free (no ethanol)". The real finding is naming: seven products carry a volatile alcohol under five different spellings, our chips use three phrasings, and eight strings now cover two ideas. The belief that cosmetic ethanol dries skin is also weaker than we assumed. A 2007 patch-test and wash study in 15 people found no significant change in water loss or redness, and a 2025 randomised half-side trial in 9 atopic patients over 30 days found a 12% ethanol cream no different from its ethanol-free control. Cetearyl alcohol, the one nobody worries about, was the most common baseline-series allergen in leave-on "hypoallergenic" products in a 2024 audit of 208 of them.

We went looking for a labelling error and didn't find one

The method took an afternoon. Pull all 54 ingredient lists, search each for the volatile alcohols (ethanol, alcohol denat., isopropyl alcohol, t-butyl alcohol), then cross-reference against the 19 products carrying an alcohol-free chip. Our lists run to a median of 33 entries and a longest of 87, so eyeballing it is a bad idea.

Two came back with something alcohol-shaped in them. The Nomadia cleansing milk lists Cetearyl Alcohol at 5 of 46 and C14-22 Alcohols at 9, both low risk, both tagged "Emollient." The Dr.Bio eco moisture cream lists Cetearyl Alcohol at 6 and Stearyl Alcohol at 14, also low risk, tagged "Emulsifier."

Zero of 19.

Neither of those is ethanol. The FDA's page on the claim is blunt about it: "Cosmetic products, including those labeled 'alcohol free,' may contain other alcohols, such as cetyl, stearyl, cetearyl, or lanolin alcohol. These are known as fatty alcohols, and their effects on the skin are quite different from those of ethyl alcohol."

The family name is nearly the only thing they share. Cetearyl alcohol, per its COSMILE Europe entry, is a mixture of cetyl and stearyl alcohol made by reducing the matching fatty acids, and it works as an emulsion stabiliser, opacifier, emollient, surfactant and viscosity controller. It is a waxy solid you could hold. Ethanol evaporates off your hand.

We amended both anyway. On 16 August the chips on those two products were changed to read "Alcohol-free (no ethanol)", because a shopper scanning 46 ingredients for the word "alcohol" hits Cetearyl Alcohol and concludes we lied. Being technically correct is a poor answer to that. It is the same fix we made after auditing our fragrance-free claims, where the problem was also vocabulary rather than honesty.

Five spellings for one idea

Seven products in the catalogue contain a volatile alcohol, and not one of them claims to be alcohol-free. The cure-alpha aloe cica soothing gel lists "Ethanol" 4th of 30. The hidiff Royal Soft Clean Kit lists "Alcohol Denat." 4th of 41. The Medi Dr.Day blue UV sun mist lists "Alcohol" 9th of 36, and the Seapuri body mist lists "Alcohol" 3rd of 32. Dr.Orga's alpine rose conditioner has "Alcohol" at 43 of 46, 8grow's pro-vitamin B5 hair tonic has "Alcohol (Ethanol)" at 2 of 46, and the YBK relief hydration light sunscreen has "t-Butyl Alcohol" at 45 of 48. All seven are graded mid risk. That consistency is real, and it is the least interesting thing here.

Look at the spellings instead. Ethanol. Alcohol. Alcohol Denat. Alcohol (Ethanol). t-Butyl Alcohol. Four are ethanol in one form or another, the fifth is a different short-chain alcohol, and a shopper reads all five as one word.

Under the INCI system, "Alcohol" standing alone means ethanol, which the COSMILE Europe dictionary states without qualification. "Alcohol Denat." means denatured ethanol, and it exists for fiscal reasons: ethanol for human consumption is taxed in the EU, so cosmetic-grade ethanol gets a denaturant added to make it undrinkable and keep it out of the duty. That is a tax rule wearing an ingredient name.

We wanted to read this off the European Commission's own CosIng database and couldn't. CosIng renders as a JavaScript application, so its entry pages return no readable text to anything that is not a browser. COSMILE Europe, run by the trade association Cosmetics Europe, is an interface onto the same INCI dictionary, so our naming source is the industry's front end rather than the regulator's.

Then there is our half of it. Sixteen chips say "Alcohol-free," two say "Alcohol-free (no ethanol)," one says "Denatured-alcohol-free." Zero products use "SD Alcohol," the American convention, which at least spares everyone a ninth string.

We are standardising on one phrasing, and the honest reason it hasn't happened is that "Alcohol-free (no ethanol)" is clearer and uglier. The hatherine nose pack is the hard case: "Denatured-alcohol-free" is our most precise chip and the least readable thing on the page.

The drying story is thinner than we assumed

We expected the 2007 and 2025 data to back the folklore, at least partly. Mostly it doesn't.

Löffler and colleagues, in the British Journal of Dermatology in 2007, ran single and repetitive patch testing with 60% to 100% ethanol on 15 people, against 0.5% sodium lauryl sulfate as a positive control, plus wash tests. Neither transepidermal water loss nor erythema changed significantly under the alcohols. Hydration did fall significantly, which is the one point in folklore's favour. And washing with alcohol came out significantly less irritating than washing with detergent (p<0.001 for water loss), with ethanol applied after washing showing a protective effect on the barrier measurements.

A 2025 paper in Scientific Reports took the harder population: nine atopic dermatitis patients, double-blind, randomised, placebo-controlled, half-side over 30 days, a moisturising cream with 12% ethanol against the identical cream without. No significant difference in water loss, stratum corneum hydration, erythema or pH. Nine people is nine people and the authors call it a pilot, so it settles nothing alone. It points the same way as the 2007 work, which is the part that caught us out.

Nine patients, 30 days, nothing measurable.

We are not telling you ethanol is good for skin, and we are not repeating the claim that it wrecks your barrier. The belief does more work in people's routines than two controlled studies support. If you skip the hidiff kit because you dislike how alcohol denat. at 4th of 41 feels, that is a preference.

Which is why those position numbers need a warning of their own. Article 19 of Regulation (EC) No 1223/2009 requires ingredients in descending order of weight at the time of addition, but everything at or below 1% may be listed in any order after that point. Korea's 화장품법 시행규칙 별표 4, amended in 2025, is the same rule structurally. So "Alcohol" at 43rd of 46 in the Dr.Orga conditioner tells you close to nothing. Fourth of 41 tells you more, and it still is not a percentage.

The alcohol nobody worries about has the better paper trail

In 2024, Hiranput and colleagues audited 208 products marketed as "hypoallergenic" against the British Society for Cutaneous Allergy baseline series. 73.6% contained at least one baseline-series allergen, and the most common one identified in leave-on products was cetearyl alcohol.

That is the ingredient that made two of our chips look wrong.

There is a case series underneath it. Nishioka and colleagues described seven patients in the Journal of Dermatology in 2022 with contact dermatitis traced to stearyl alcohol in topical medications, all patch-test positive, and 2 of the 3 also tested with cetyl alcohol reacted to that too, suggesting cross-reactivity inside the fatty alcohol family. Stearyl Alcohol sits at position 14 in our Dr.Bio moisture cream, a leave-on.

What we could not find is the number that would put this in proportion. There appears to be no general-population patch-test positivity rate for cetearyl or cetyl alcohol, none we could locate anyway. The 4.4% figure that circulates comes from a leg-ulcer population, where broken skin and years of topical treatment make sensitisation far likelier. Stretching that to everyone reading this would be worse than saying nothing, so we quote no rate at all.

Twelve of our 54 products contain a fatty alcohol and we are not about to start flagging them. Our screening grades cetearyl and stearyl alcohol low risk, and that grade is right for almost everyone. But the audit of 208 products and the 7 cases from 2022 say something the popular version of this topic has backwards: if a rich cream has burned you and you never worked out why, the emulsifier is a better suspect than the ethanol.

What our screening found

Both products below carry something we should put in front of you before any praise. The Nomadia cleansing milk is one of the two whose chip we amended on 16 August: Cetearyl Alcohol at position 5 of 46, C14-22 Alcohols at 9, precisely the ingredient the 2024 audit picked out, though this one rinses off. The hidiff kit has Alcohol Denat. 4th of 41 and makes no alcohol-free claim, which is our labelling working as it should, but we have not settled whether that product is leave-on or rinse-off, and the classification changes how much a 4th position matters. That gap in our own data is unresolved today. Catalogue-wide: 19 alcohol-free chips, 0 conflicts, 7 products carrying a volatile alcohol and none claiming otherwise, 12 carrying a fatty alcohol. The labelling is consistent. The vocabulary is not, and unlike the unregulated words we picked apart in clean, vegan and non-comedogenic, this one is ours alone to fix.

Two fatty alcohols sit high in a 46-ingredient list, one of them the most commonly identified allergen in leave-on hypoallergenic products, so anyone with a known fatty-alcohol reaction should skip it. It is a rinse-off cleansing milk, which limits contact time, and it scores 9.7.

The practical takeaway

Treat "alcohol-free" as a claim about one molecule. It means no ethanol and no denatured ethanol. It does not mean no cetearyl alcohol, no stearyl alcohol, no lanolin alcohol, and the FDA has said so in writing for years, which puts it in the small group of claims that carry a definition.

If you have avoided ethanol because you were told it damages your barrier, the two controlled studies we could read (15 people in 2007, 9 atopic patients in 2025) do not support that. Avoid it for feel if you want. Sting, tightness, the smell of a body mist with alcohol 3rd of 32. Those are reasons enough and need no biology behind them.

And if you react to creams rather than toners, invert the usual advice and read for the emulsifier first. Cetearyl alcohol at position 5 gets more contact time than an ethanol at 43 of 46 ever will. A spreading rash, or one that won't settle, belongs with a dermatologist and a patch test rather than a label audit.

Our own job is smaller: one phrasing on the chip instead of three, and a decision on whether the hidiff kit is leave-on. The criteria we screen against count naming consistency in the score, which is why finding nothing wrong with 19 labels still counts as a bad result.
